Housing Stabilization Services
We assist individuals with disabilities, including mental illness, substance use disorders, and seniors, in finding and maintaining stable housing. Services include housing consultation, transition, and sustaining support.
Housing Consultation
Services Provided:
- Assisting with documentation required for Housing Stabilization eligibility.
- Creating a person-centered housing plan based on assessment outcomes.
- Identifying housing needs, preferences, and cultural requirements.
- Supporting informed choice of housing transition or sustaining providers.
- Offering resources for non-housing goals identified in the person-centered plan.
- Coordinating with other service providers.
- Educating on privacy rights and appeals.
- Annually updating the housing plan.
Services not Covered:
- Person-centered plans for individuals with Medicaid-funded case management or who are ineligible for HSS.
- No-show appointments.
- Documentation tasks after plan completion.
- Assessments
Limitations
- Services must be provided by a Medical Assistance-enrolled provider.
- Not available for individuals with Medicaid-funded case management (e.g., mental health or elderly waivers).
- Consultation must be tied to settings that meet HCBS (Home and Community-Based Services) requirements.
- Limited to one session annually unless specific circumstances require updates.
Housing Transition
Services Provided:
- Developing a housing transition plan.
- Supporting benefits applications to afford housing.
- Assisting with housing search, tenant screening, and applications.
- Providing budgeting support and lease education.
- Facilitating landlord relationships and cultural understanding.
- Organizing moves and finding funding for deposits or home goods.
- Ensuring new housing is safe and ready for move-in.
Services not Covered:
- Expenses like rent, deposits, or furnishings.
- No-show appointments.
- Services for individuals transitioning from institutions if they pass away before discharge.
Limitations
- Limited to 150 hours per transition, with exceptions for additional hours if approved.
- Services must focus on less restrictive housing settings.
- Not available for individuals concurrently receiving Relocation Service Coordination or ACT services.
Housing Sustaining
Services Provided:
- Regular updates to housing and safety plans.
- Early identification of issues that jeopardize housing.
- Coaching on tenant responsibilities and lease compliance.
- Advocating with landlords or community resources to prevent eviction.
- Supporting benefits retention and income maintenance.
- Building community and cultural support networks.
Services not Covered:
- Expenses like rent, deposits, or furnishings.
- No-show appointments.
- Services provided in group settings or outside Minnesota.
Limitations
- Limited to 150 hours per transition, with exceptions for additional hours if approved.
- Must be provided in approved housing settings.
- Not available for individuals concurrently receiving Relocation Service Coordination or ACT services.
